Description

Good morning everyone,

  There is a severe crash happening with the file upload question type.

  In version 4.2.5 + 200526, when a "file upload" question is presented, after choosing the file, a blank screen appears, and the file is not loaded. Thus, it is impossible to continue answering the questionnaires.

  I have tried to create different questionnaires in two separate installation instances. However, the problem occurs in both cases.

  Is it possible to solve the problem with some urgency, since this problem causes the system to become inoperable?

Steps To Reproduce

1) create any questionnaire, with a "file upload" question.

2) when trying to upload, the system redirects the user to a blank page.

Yes, it is chromium 83.0.x . We will check it next week (unless someone has time to make a PR for that).

We solved it only for LS 3 (learned about it yesterday as well, and took a bit of time till we found the culprit)

Working as Expected,able to successfully add file in Chrome 83 Version,Firefox.Please refer the screenshot for more details.
