Although the pictures I attached it at the 2.00 product version, I downloaded the latest version (2.05) just yesterday (Jan 27, 2014) and the bug is still there. Here is way to reproduce the bug:

(1) Create a survey, then create a question group, then create a question (by clicking the + button). Once we do this, basically we are in "1st-screenshot" (refer to my attachment). Note the list of questions within the big red oval.

(2) Now, change the "Question type" to other type of question type: anything other than "List (drop down)".

(3) Change the "Question type" again to "List (drop down)", the list of questions underneath will be different, refer to "2nd-screenshot", notice the purple big oval.

It seems that the value set within function OtherSelection (application/views/admin/survey/Question/questionJavascript_view.php) is not read the first time we hit the "+" (add a question) button. However, once we start selection the question type using the drop down, the correct values are set.
